Why Iron Is Hard on a Stomach, and What Changes It

Ferrous sulfate roughly doubles the odds of stomach trouble against placebo. What the same trials show is that the dose is not the lever most people assume, and the schedule may be.

The question rarely comes up before the first bottle. It comes up around day four, in a message that always has the same shape: is this normal, and does it stop. Iron has a reputation for being hard on a stomach, the reputation is earned, and the research that earned it also says something less obvious about what changes it.

What forty-three trials measured

A 2015 systematic review and meta-analysis in PLoS One pooled 43 randomised controlled trials covering 6,831 adults, comparing ferrous sulfate against either placebo or intravenous iron. Against placebo, ferrous sulfate raised the odds of gastrointestinal side effects with an odds ratio of 2.32 (95% CI 1.74 to 3.08). Against intravenous iron, the odds ratio was 3.05 (95% CI 2.07 to 4.48).

Roughly double the odds against a dummy capsule. That is the honest size of the thing: not universal, not trivial, and not in anyone's head.

Two cautions belong with those numbers. Heterogeneity between the trials was moderate, 53.6 percent against placebo, meaning the studies did not all agree closely. And a subgroup of seven trials in pregnant women showed a higher odds ratio, 3.33, but with a confidence interval running from 1.19 to 9.28 and heterogeneity of 66.1 percent. A range that wide is a signal that the pooled figure is carrying a lot of variation, not a precise measurement.

Where the discomfort comes from

A capsule of iron delivers more iron than the intestine will take. Absorption is capped, and what is not absorbed does not vanish. It continues down the tract, and iron is a reactive metal for the whole of that journey. The discomfort is not a reaction to the idea of a supplement, it is the fraction that never crossed the wall.

That framing matters because it points at the two variables worth examining: how much iron is presented at once, and how often. Our piece on chelated iron and iron salts covers the third variable, which compound the iron arrives attached to.

The dose does not behave the way you would expect

Here the evidence turns awkward, and it is worth staying with the awkwardness.

The obvious hypothesis is that a bigger dose causes more trouble. The 2015 meta-analysis tested it directly and did not find it. Its meta-regression produced no significant evidence of an association between a study's odds ratio and its iron dose. The authors state the conclusion plainly: ferrous sulfate is associated with a significant increase in gastrointestinal side effects, but they do not find a relationship with dose.

That does not mean dose is irrelevant to everything. It means dose did not explain tolerability across those trials, and anyone claiming a clean dose-response curve for side effects is claiming more than this evidence supports.

What dose does govern is yield, and it governs it with sharply diminishing returns. A 2015 study in Blood gave labelled iron to 54 young women with plasma ferritin at or below 20 µg/L. Increasing the dose sixfold, from 40 mg to 240 mg, increased the iron actually absorbed only threefold, from 6.7 mg to 18.1 mg. Fractional absorption fell as the dose climbed.

The same study found the mechanism. Twenty-four hours after a dose of 60 mg or more, serum hepcidin was raised and fractional absorption was down by 35 to 45 percent. Hepcidin is the hormone that closes the gate. A large dose does not only fail to deliver proportionally more iron, it partially shuts the door on the next one.

Less often, rather than less iron

If a dose keeps the gate shut for about a day, the interesting question is not how much but how often. A randomised, double-blind, placebo-controlled trial published in EClinicalMedicine in 2023 put it to the test in 150 Swiss women with serum ferritin at or below 30 µg/L.

One group took 100 mg of iron daily for 90 days. The other took the same daily dose on alternate days for 180 days. Same total iron, different spacing.

Ferritin ended in the same place, and the difference was not close to significant. Side effects on the days iron was actually taken were markedly more likely on the daily schedule. Hepcidin sat lower on the alternate-day schedule, which is the mechanism doing its work. And at six months, deficiency had returned in 11.4 percent of the daily group against 3.0 percent of the alternate-day group.

The finding is counterintuitive enough to be worth stating twice: taking the same amount of iron less frequently was better tolerated and, by six months, left fewer women deficient.

Black stools are not a side effect

Oral iron darkens stools. It is the unabsorbed fraction leaving, it is expected, and it is not a sign that anything has gone wrong.

What is not in that category is a black, tarry stool arriving with abdominal pain, dizziness, or unusual fatigue. That combination is not a supplement question and it is not one for an article to settle. It belongs to a doctor, promptly.

The part that does not flatter us

We sell an iron formula built on a chelated compound, so the tidy story would be that chelation solves this. The evidence does not hand us that story cleanly, and three things cut against it.

First, a 2024 paper in J Pregnancy reporting results from two randomised studies found that low-dose prophylactic oral iron in pregnancy was not associated with clinically significant gastrointestinal complaints, and that held for ferrous fumarate, ferrous bisglycinate and ferrous sulphate alike. At low prophylactic doses, in that population, the compound was not the deciding variable. A brand that tells you its form is gentler at every dose is going further than this evidence goes.

Second, the meta-analysis above found no dose relationship for side effects. Any simple narrative of the form "less iron, gentler experience" is not supported by the pooled trial data, however intuitive it sounds.

Third, and most directly against our own protocol: the strongest recent evidence on tolerability favours alternate-day dosing, and our guide asks for two capsules every day. That is a real tension and we would rather name it than hide it. The alternate-day trial used 100 mg of iron in women already depleted, a treatment context rather than a maintenance one, and its schedule question has not been settled across doses or populations. But the honest sentence is that the schedule is a live question, not a closed one, and anyone whose stomach objects has a conversation worth having with a clinician rather than a rule to obey from us.

When it is not a side effect at all

Some things sit outside this subject entirely. Severe or persistent abdominal pain. Vomiting. A black tarry stool with pain, dizziness or unusual fatigue. Shortness of breath. Symptoms that arrive alongside heavy bleeding. None of those are tolerability questions, and all of them are reasons to speak to a doctor rather than to adjust a capsule.

Anyone taking a prescribed iron product, a proton pump inhibitor, or any regular medication should raise the subject with their prescriber before adding anything, and that is not a formality.

Where this lands on a label

The Blood study's own conclusion was that providing lower dosages, in the range of 40 to 80 mg of iron, and avoiding twice-daily dosing, maximises fractional absorption.
