Two bottles, both labeled iron. One costs a few dollars. The other costs several times that. Turn them over and the ingredient lines diverge: one says ferrous sulfate, the other says ferrous bisglycinate, or sometimes just chelated iron. Neither bottle explains why that word should matter to your stomach. It does matter, and the reason is mechanical rather than mysterious.
Iron in a capsule is always iron plus a partner
Iron is a metal. You cannot put metal filings in a capsule and expect a digestive system to make use of them. So every oral iron product pairs iron with a partner molecule that carries it. The partner is the variable. The iron atom is not.
That partner decides two things. First, how much elemental iron actually rides in each dose, since the compound weight and the iron weight are different numbers. Second, what state the iron is in when it reaches the small intestine. The second one is where tolerance is decided.
What an iron salt is
Ferrous sulfate, ferrous gluconate, and ferrous fumarate are the three names you will meet most often. They are the oldest and cheapest options on the shelf, and they are ionic salts. The iron is held to its partner by electrical charge, not by a stable structure. Elemental iron content varies by salt: ferrous sulfate runs around a fifth of the compound by weight, ferrous gluconate less, ferrous fumarate more.
In the acid of the stomach, that charge pairing comes apart. Iron is released as a free, charged ion. From there, most of it that gets absorbed goes through a protein in the wall of the small intestine called DMT1, the divalent metal transporter. DMT1 is a door with a capacity. Doubling the dose does not build a second door.
What chelated actually means
Chelate comes from the Greek word for claw. In ferrous bisglycinate, two molecules of glycine (the smallest amino acid there is) close around the iron atom and hold it inside a ring. That is a structure, not a charge pairing. It holds together through stomach acid better than a charge pairing does.
Here is where we will be more careful than most pages you will read on this. Whether the intact chelate crosses the intestinal wall as one piece, or whether the glycine lets go at the brush border and DMT1 takes the iron from there, is not settled. Cell work published in Nutrients, using intestinal cells with DMT1 knocked out, found that iron from ferrous bisglycinate was probably transported mainly through DMT1, much like ferrous sulfate. The intact route is asserted often on product pages. A clean percentage on that question is a number nobody has earned yet.
The same caution belongs on the tolerance question. A 2022 review of oral iron treatment in adult iron deficiency notes that although chelated iron may be less affected by enhancers and inhibitors of absorption than iron salts are, clinical data has not shown chelated iron to be superior to iron salts. We are quoting that rather than hiding it, because a brand that only cites what flatters it is not worth reading.
What is less disputed is the part before the wall. For as long as the iron stays inside the glycine ring, it is not sitting loose in the gut as a reactive ion. That distinction carries the next section.
The iron you do not absorb does not sit there quietly
Only a fraction of any oral iron dose crosses into the body. The remainder keeps traveling down the digestive tract. It is not neutral cargo.
That same review is direct about the consequence. Unabsorbed iron propagates many of the side effects that complicate oral iron use, including dyspepsia and constipation. Free iron in the intestinal lumen is chemically reactive. It interacts with the lining it passes and with the bacteria living there. Dark or black stool is simply the visible trace of iron that never made it in.
This reframes the cheap bottle in a way the price tag hides. Raising the dose of an iron salt does not raise absorption in proportion, because the door has a capacity. Past that capacity, the extra iron is not delivered. It is only present. And present is exactly the condition that produces the symptoms.
Tolerance is a property of the compound, not of the person
Oral iron gets abandoned often, and the reason written down afterward is rarely the whole story. The published lists of side effects are not exotic: nausea, vomiting, constipation, diarrhea, epigastric distress, and a tarry black stool. They are ordinary enough that a person can decide the difficulty was her own resolve.
Look at what that account leaves out. The capsule was taken. It kept being taken until continuing became unreasonable. What changed was not the will of the person holding the bottle. What changed was how much unabsorbed iron was moving through a gut every day, and that is a property of the compound in the capsule. Discipline was never the variable under examination.
That distinction carries weight with iron specifically, because a reserve is not a tank you top off over a weekend. Refilling a reserve is slow work, which is the subject of how long it takes to refill an iron reserve. A form you cannot stay on long enough to matter is the wrong form, whatever it costs per capsule.
How to tell which one you are holding
The information is on the bottle. It is just rarely on the front.
- Find the word after the word iron. A front panel that says Iron 18 mg gives you a quantity and withholds a form. The form usually appears in parentheses in the ingredient line, or in a footnote below the Supplement Facts panel.
- Sort what you find. Ferrous sulfate, ferrous gluconate, ferrous fumarate: salts. Ferrous bisglycinate, iron bisglycinate chelate, ferrous bisglycinate chelate: a chelate. If the label says amino acid chelate without naming which amino acid, that is a vaguer statement than it looks, and it is fair to ask why.
- Read elemental iron, not compound weight. The panel is supposed to state elemental iron. When a marketing headline and the panel disagree, the panel is the number that counts.
- Then read the rest of the lines. Getting iron across the intestinal wall is one step of three. Absorbing it, converting it, and storing it are separate jobs, and iron alone only addresses the first. What a label does about the other two tells you what it was designed for.

What we put on our own four lines
The Reservoir Formula uses chelated iron bisglycinate rather than an iron salt, for the reason laid out above: less free ionic iron traveling through a gut that has to tolerate it every morning over a long stretch. Alongside it sit 100 mg of lactoferrin and the cofactors in their active forms, methylcobalamin, L-5-MTHF, P5P, vitamin C, and copper, because absorbing iron is only the opening step of the three. Read the panel. Compare it against whatever else you are considering. We would rather you check the label than take our word for it.
