You are holding two bottles. Both say iron. Both say gentle. One costs four dollars and one costs forty, and the front of the box will not tell you which is which. The front is written by a marketing team. The small panel on the back is not, and it takes about thirty seconds to read once you know which four lines matter.
This article is that test. It works on any iron bottle in any store, including ours. It does not ask you to trust a brand. It asks you to read four lines and see what they actually say.
Where the four lines live
Every dietary supplement sold in the United States carries a Supplement Facts panel, the boxed table with a heavy rule under the heading. Vitamins and minerals come first, each with an amount per serving and a percent Daily Value beside it. Below them sits a second group, the ingredients that are not vitamins or minerals, listed without Daily Values. Below the whole box, in running text, are the "Other ingredients": capsule shell, flow agents, colors.
Everything you need is inside or just under that box. The four lines, in reading order, are the form, the elemental dose, the lactoferrin, and the cofactors. Each answers a different question about whether the iron in the bottle ever reaches the reserve it is meant to fill.
Line one: the form
Look at the word in parentheses after the word Iron. That is the form, and it tells you what the iron is bound to before you swallow it.
Two families dominate the shelf. The salts: ferrous sulfate, ferrous gluconate, ferrous fumarate. And the chelates, usually printed as ferrous bisglycinate or iron bisglycinate chelate, where the iron is held between two glycine molecules. The salts are old, cheap, and genuinely capable of delivering iron. The documented adverse effects of oral iron are mostly gastrointestinal: nausea, vomiting, constipation, diarrhea, epigastric distress, and a tarry black stool. That is a tolerance question rather than a potency question.
If the panel does not name the form, do not assume it is hidden. The source is often printed in the running text under "Other ingredients" instead, so look there first. A bottle that names its form in neither place has told you the least it is allowed to tell you. Line two: the elemental dose
Iron the mineral weighs less than the compound carrying it. Ferrous sulfate is roughly twenty percent iron by weight. Ferrous fumarate is roughly thirty three percent. Ferrous gluconate is roughly twelve percent. So 325 mg of ferrous sulfate is not 325 mg of iron.
The panel is built for this. The rule is that the declared weight represents the dietary ingredient itself and not its source: the weight of calcium rather than of calcium carbonate, the weight of iron rather than of the salt around it. So the figure standing beside the word Iron, the one carrying a percent Daily Value, is the elemental dose. That is the number to write down.
The front of the box is free to print the larger compound number, because larger reads better. When the front and the panel disagree, the panel is the side under rules. A number printed with no unit at all is not a dose, it is a graphic.
Line three: lactoferrin, named or named and dosed
Lactoferrin is not a vitamin and not a mineral. It is a protein, so on a Supplement Facts panel it belongs to that second group below the vitamins, the one without Daily Values. That placement matters more than it looks, because that group is the one that can legally be folded into a proprietary blend.
Inside a blend, only the total weight of the whole blend has to be declared. The individual ingredients are then listed underneath it in descending order of predominance by weight, with no amounts of their own. A label can therefore name lactoferrin, print it large on the front of the box, and never once tell you how much is in the capsule.
The check is a single look. Is there a milligram figure on the same line as the word lactoferrin? If yes, it is dosed. If the word sits in a list under a blend heading with one total below it, it is named and not dosed, and those are not the same claim. Use the ordering, too. Since the list runs heaviest to lightest, a name near the bottom of a blend weighs less than every name above it. An ingredient sitting last inside a blend that totals 200 mg can be present in a trace.

Line four: the cofactors, and the conversion step
A reserve is not filled by iron alone. Absorb it, convert it, store it: three steps, and the later ones lean on other nutrients. This is where two labels can look identical and read differently, because the difference is not in the amount. It is in the spelling.
Three pairs are worth recognizing on sight:
- Folate. Printed as folic acid, or as L-5-MTHF (also written 5-MTHF, L-methylfolate, or 5-methyltetrahydrofolate). Once absorbed, folate is enzymatically reduced to DHF, then to THF, and ultimately to L-5-methyl THF, the form that predominates in circulating plasma. The second spelling starts further along that road.
- Vitamin B6. Printed as pyridoxine hydrochloride, or as pyridoxal 5-phosphate, often shortened to P5P. Pyridoxal 5-phosphate is an active coenzyme form of vitamin B6, and vitamin B6 is metabolized at the cellular level into that form.
- Vitamin B12. Printed as cyanocobalamin, or as methylcobalamin. These are two different compounds, and the panel is the only place that tells you which one is in the capsule.
None of this makes the plain forms worthless. A label that uses them is still telling you exactly what is inside. The point here is narrower. The longer spelling is not decoration, it is a different chemical entry point, and it costs more to put in a capsule. A brand that paid for it will name it exactly. A brand that did not will print "Folic Acid" and stop. Give vitamin C and copper the same glance while you are there, since both belong to the same iron handling story.
Run the test on us first

- Find the form. Named in the panel, named under "Other ingredients", or named nowhere.
- Find the elemental figure beside the word Iron, the one carrying a percent Daily Value. Ignore the front of the box.
- Find lactoferrin and look for a milligram figure on its own line. Named is not dosed.
- Read the spelling of the folate, the B6, and the B12.
Four answers, and you can sort most of a shelf with them. You can also sort us.
